For the convenience of the Kaleida Health CEOC please use this form to secure and pay for both your Executive Table and your Employee Appreciation Table. If you know the guests for your Executive table now you can enter them while submitting the order. Alternatively you will be sent a confirmation code with a link to update your guests at any time or can email a list to kaleidafoundation@kaleidahealth.org up to one week prior to the ball. 

Employee appreciation table will be filled by lottery.
